The plan should include changes you want to make:
- your life style
- things to achieve
- things that need to stay the same
- where you live
- long term.
The plan will not be agreed if it is:
- not clear
- changes are imposed by others
- changes would make life worse.
Question 3: How will you be supported?
The plan should include the help you need:
- when and where
- staying safe and well
- managing risks.
The plan will not be agreed if :
- there is no detailed plan for support
- the plan puts you or others at risk.
Question 4: How will you use your personal budget?
The plan should include a detailed breakdown of the costs of your support plan.
The plan will not be agreed if you don't say how your money will be spent.
Question 5: How will your support be managed?
The plan should include how your support will be managed:
- contingency plans
- how will the money be managed
- details of how you will directly employ people if needed.
The plan will not be agreed if you don't say how your day to day support will be managed.
Question 6: How will you stay in control of your life?
The plan should include how you make decisions.How are you involved in decisions if other people help you make them?
The plan will not be agreed if others make decisions that you can make.
Question 7: What are you going to do to make this happen?
The plan should include a clear action plan of who is going to do what.
The plan will not be agreed it isn't clear how the support will be delivered.
