Shared care protocols

Shared care protocols are used when individuals living in their own homes in the community require health and social care services from the Primary Care Trust and Social and Community Services to be delivered by providers of domiciliary care services. The protocols are intended to clarify the boundaries of roles and responsibilities and the process that must be applied when providing health and social care services to people in Oxfordshire. By working together, health and social care services will be provided in a coordinated, effective and client centred way, making best use of available resources.

The protocols are aimed at council staff, the providers of domiciliary care services and their home support workers, The Primary Care Trust’ district nurses and other healthcare professionals, working with clients of Oxfordshire County Council Social and Community Services and the Primary Care Trust.
